How to Request Medical Records

Patient Requests

Catawba Valley Medical Center has partnered with Sharecare to fulfill your requests for records. Catawba Valley Medical Center is committed to protecting your medical information. For information about your rights and the obligations you have regarding the use and disclosure of your medical information, please see our Notice of Privacy Practices.

If you are our patient and would like to request your medical records, please click on the link below to complete your request for medical records. You will be required to provide a valid email address and a government-issued ID.

If you are unable to complete the electronic form above, download and complete the paper form below. The authorization form must be legible and complete. Authorizations that are illegible or incomplete will be returned. Please note the processing of records is faster using the online form above.

Records are usually available within 10 days from the time the request is received. To arrange for another individual to pick up the documents for you, please indicate that on the authorization form, and ask them to bring their government-issued ID.

Only the patient, parent/legal guardian, or the patient’s legal health care representative can sign the form to release medical records. If you are requesting records on behalf of the patient or as the patient’s representative, please provide a copy of an Advance Directive/Durable Power of Attorney for healthcare/ Conservatorship.

If you are an attorney, insurance company, or any other entity requesting records from our facility, please click on the link below to upload your request along with the patient’s authorization.

Continuity of Care Requests

Continuity of Care requests requiring urgent attention may be faxed to 828-326-2910. Please indicate  “STAT” on the request to ensure it is prioritized accordingly.

How can I contact the Medical Records Department directly?

The Medical Records Department can be reached at 828-326-2464. For assistance, please reach out during our normal business hours, Monday through Friday, from 8:30 a.m. to 5:00 p.m. If contacting outside of business hours,

please leave a voicemail with your name and call back number and we will return your call the next business day.

Please note that hours of operation may vary on certain recognized holidays.

When will my online medical record request be ready?

Your records will be ready in 3-5 business days from the receipt of your electronic request.

How do I request an amendment to my medical record?

Amendment requests can be mailed to Catawba Valley Medical Center 810 Fairgrove Church Rd. SE Hickory, NC 28602 Attn: Privacy Supervisor – HIM Department or sent via e-mail to myhealth@catawbavalleymc.org

After your Amendment Request has been received it will be forwarded to the treating provider. The provider will then review the documentation to determine if an amendment is appropriate, and once a decision has been made you will be notified. Approval or denial of any amendment is at the provider’s discretion. Call 828-732-7127 for questions.
