How to Request Medical Records

1. A valid and complete Authorization for Release of Health Information Form signed and dated by the patient is required to request medical records. If needed, download and complete the Release of Health Information Form.
2. Return the completed form with one of the following options:

• To obtain a copy of your records in person, come to the Fairgrove Profession Building (located between the hospital and the credit union). Medical Records is on the first floor of the building. Be prepared to show a photo ID to obtain the records. The office hours are Monday- Friday 8:30am- 5:00 pm.
